Consents & Waivers

These forms must be completed before registering with Softball BC. This applies to all participants, players, coaches, umpires, executive members, team managers - anyone listed on a team roster and/or requiring insurance coverage from Softball BC. These forms are to be submitted to your local association/club registrar or President. Please contact your local association for more information on where to submit these forms.

CONSENT FOR USE OF PERSONAL INFORMATION AND PHOTO RELEASE
INFORMED CONSENT AND ASSUMPTION OF RISK AGREEMENT MINOR (to be signed on behalf of the participant who is under the age of 19 as of signing)
INFORMED CONSENT AND ASSUMPTION OF RISK AGREEMENT 19 (to be signed by participants such as coaches and players who are 19 and older as of the date of signing)

Coach Feedback & Complaints

Coaches can submit complaints or feedback about umpires using the dedicated form below.
Submissions are automatically directed to the District UIC for review and record-keeping.

Incident & Ejection Reports

If an incident or ejection occurs, umpires are required to file a report.

Reporting Guidelines:

Incident & Ejection Report Form

A separate report must be submitted for each person ejected.

Reports should be submitted as soon as possible (within 48 hours)

The umpire responsible for the ejection must contact their DUIC on the day/evening of the game.

Within 48 hours, the umpire must complete and submit the Incident & Ejection Report.

The Provincial Umpire-in-Chief will review the report and forward it to your DUIC.
