Incident/Ejection Report

SOFTBALL BC UMPIRE INCIDENT & EJECTION REPORT

Use the form below to submit an incident and/or ejection report.

Submit a separate report for each person ejected.

Reports should be submitted as soon as possible after the incident (no later than 48 hours).

The umpire responsible for the ejection shall contact their DUIC the day or evening of the game to report the incident.

Within 48 hours of the game, the umpire shall complete the Umpire’s Incident & Ejection Report and submit the form.

The Provincial Umpire-in-Chief will review the form and forward it to your DUIC.
